Food Service Meal Delivery Request Form
All meal deliveries are subject to current household meal benefit status.
One meal pack per student per week.

The meal benefit application can be found on the Food and Nutrition page under National School Lunch Program.

A meal pack includes 5 breakfast meals and 5 lunch meals. The price for the meal pack is based on the student’s meal benefit status and listed below.

Meal delivery service will begin the week after the form is submitted and cease when in person school resumes. Students/families still have the option to utilize the curbside pick-up which will also be based on the student’s meal benefit status. Deliveries will occur between 7:00 am and 12:00 pm, parent/guardian/student must be home to receive meals.

Monday: Tolleson and University High Schools
 
Tuesday: Curbside pick-up
 
Wednesday: Westview and Copper Canyon High Schools
 
Thursday: La Joya and Sierra Linda High Schools
 
Friday: West Point High School
